You can download the visa application forms below accordingly to your applicable visa type. To open these documents, you will need Adobe Reader.
1. Letter of authorization for visa application/passport return
Mandatory if you choose to submit your application/collect your passport by a representative. The letter cannot be sharable and each application must have one original separately. Please be aware that two originally signed letters of authorization are mandatory for submitting your application and collecting your passport by a representative and a photocopy is invalid.
If the Dutch sponsor lives in China but invites the applicant to travel together to the Netherlands for visit family/friend purpose.

In order to protect the security of your personal information, all the applicant need to download, print, complete and sign the Statement of consent for personal information processing and cross-border transfer and submit it to the Visa Application Center when submitting your visa application materials, so as to provide you with corresponding services.

The applicants need to go to “Consular services porta - Schengen Visa” of Dutch MFA to fill out and print the visa application form online before submitting the visa application. The application form has to be signed by the applicant. Minor applicant under the age of 18: The application form must be signed by both of parents or by a legal guardian. Please DO NOT sign the name of underage applicant.
2.Caribbean area visa application form
The applicants need to go to “Consular services portal - Caribbean Visa” of Dutch MFA to fill out and print the visa application form online before submitting the visa application. The application form has to be signed by the applicant.
